Congratulations and thank you for making the right decision to participate in the Certification Audit Program as an Assured Performance On-site Inspector. Below is the official enrollment form to become an on-site inspector for Assured Performance. Please complete the form with the information requested. Once the form is received and processed, you will be contacted by an official of Assured Performance to create your electronic territorial map and provide you with the supporting credentials.

NOTE: Acceptance of the terms and conditions of this agreement as illustrated by your signature is a pre-requisite to allowing access to the Inspection site and Certification Inspector Audit information. You are agreeing to the following:

You hereby acknowledge that the alteration or falsification of documents or photograph's is strictly forbidden. Violations will result in termination of your participation as an Inspector in the Assured Performance Certification Program.

You acknowledge and understand that all the Inspections will be rated internally and also by the shop you are inspecting. You will be rated on your knowledgeable and professionalism. Top ratings will continue your assignment opportunities with the Assured Performance Network

A completed W9 must be submitted to us yearly, before receiving payment.

Assured Performance Network will process payments within 15 days from the date of completion.

“You acknowledge and understand that every inspection is strictly confidential and no information should be shared at any time with anyone other than the Assured Performance Network. Your agreement is strictly between you and the Assured Performance Network.”

You agree to keep all Assured Performance Network/Co-op Agreements confidential.
